The shoulder trades stability for range, which is why it is so easily irritated by overhead work, swimming and diving in Hurghada, or by a period of immobility. Frozen shoulder, rotator cuff irritation and subacromial pain all limit reaching overhead but behave quite differently under testing. Establishing which one you have determines whether treatment should push range, calm irritability, or load the cuff progressively.

Shoulder examination and treatment

We measure active and passive elevation, abduction and both rotations, since the gap between active and passive movement is the clearest clue to the diagnosis. Cuff strength is tested in isolation for each tendon, and scapular control is observed as you lower your arm. Treatment includes glenohumeral and scapulothoracic mobilisation, capsular stretching for genuine stiffness, cuff loading with bands and dumbbells, and shockwave where a calcific deposit is present. Sessions run 45 minutes and include sleeping-position advice, which strongly affects night pain.
